The best type of feedback you can get is candid feedback.
Whenever I or someone I work with decides to move on from a company I reach out to request feedback on how I can improve. I have found that as your regular interactions with these people come to an end they are more likely to be very open about the things you can do to improve.
Below are the questions I ask.
